At Metro Health Care Services our personal care assistant services in Minnesota mean that your loved one will always have the assistance they need for their daily living activities and their well being, whether practical issues or for health-related functions.
This ensures that your loved one can maintain their independence while still receiving compassionate, professional care.
What Does a Personal Care Assistant Do?
- Live in Minnesota
- Be eligible to receive PCA services under their Minnesota Health Care Program
- Be able to make decisions regarding their care or have someone able to make decisions for them
- Schedule an assessment with a Public Health Nurse (PHN) from their county
- Require constant supervision or hands on assistance to complete at least 1 ADL or have Level 1 Behavior
Should your loved one meet these requirements they will receive an assessment of their eligibility, daily needs, and health, followed by the creation of a care plan, and actually selecting an agency through which PCA services will be administered.
If your loved one qualifies for a Personal Care Assistant, Metro Health Care Services can provide the assistance you need
